Insurance and Payment Information

We participate with most major insurance companies, see the link below to view a list of insurance we do and do not participate with. If we are not contracted (considered OUT of network) with your insurance company, or if you have Medicare, we ask for payment in full at the time of service. We encourage all patients to confirm their coverage prior to making an appointment.

Co-payments are due at the time of your appointment along with supplies not covered by insurance companies. You will also be responsible for any unpaid balance, once your insurance company has processed your claims.

We are happy to answer any questions regarding your appointments or insurance! Insurance coverage and benefit questions, however, should be directed to your insurance company’s customer service department (number found on your insurance card) or employee benefits/human resources department at your work place.
